Google says to submit both domains with and without www. 
Do I create a new sitemap for each version of my domain?
If so, how do I do that?

Hello,

you can specify corresponding domain as Starting URL for sitemap generator.

The server didn't respond in time.

in this case I would recommend to run generator in command line if you have ssh access to your server.

The command line to be use in ssh can be found on the Crawling page of sitemap generator, example:
