The documentation says one can edit this template: pages/sitemap_tpl.html

I don't have this file.

Where is is, or where can I get one?

It's located in the folder where you have installed generator. You should open it via FTP, there will be a subfolder called "pages", and sitemap_tpl.html file inside of it.

Strange  my sitemap_tpl.html  file is in:
/xml-sitemaps/pages/mods/sitemap_tpl.html
not
/pages/sitemap_tpl.html

?

Yes, in latest version of sitemap generator location of html template has been changed.
