html sitemap - titles not displaying correctly

The HTML sitemap does not display the titles correctly.  Some pages, shown below, display page name such as group.php, authors_corner.php, blob.php, and mail.php.  The pages all have correct titles coded and verified with SEO tools.  What is the fix to display correct titles?

Html titles now showing but the root URL is now missing!! Major problem! Why is the root url missing?

Starting URL was changed to /groups ... not correct.  Changed starting url to root ( / ) and now html titles are wrong.  What is wrong?
