Missing pages in HTML sitemap

There are missing pages in the HTML sitemap that is generated.  Here is the web site I am trying to index:

[ External links are visible to forum administrators only ]

And here's the XML map that is generated:

Specific example of a missing page (from sitemap) with a link right off the index page:
