Duplicate video titles in sitemap

I am having an issues with generating video sitemaps. Google is reporting that there are duplicate video titles in the video sitemap. All of the videos are from YouTube, so I went ahead and changed all of them from the old flash format to the new HTML5 format and am still getting the issue. Also, every time a run the generator, it comes up with a different amount of videos. Any ideas on how I can get this issue resolved?

do you have different titles/descriptions defined for each video on youtube?

Basically, I try to keep the names the same as the blog that I embed them into. And I do have a description that is similar to the adjoining blog post. But I keep the keywords the same.
